Select Dose - 1st / 2nd (Provide Date if 2nd Selected)
Once registration is done, same cannot be withdrawn, due to any reason.

Strictly NO ON-SPOT Registration or WALK-INs will be allowed.

It is mandatory to register on COWIN (https://cowin.gov.in) or through AROGYA SETU App. The COWIN registered mobile number must be carried for the vaccination.

This is required for the OTP that would be generated.
